About the healthcare transition guide
This guide covers the process of an autistic person transitioning from pediatric medical care to adult medical care. It is designed for transition-aged autistic youth and young adults (14 to 21 years old), their parents/caregivers and autistic adults seeking new or age-appropriate medical providers. The goal is to provide information and resources to start planning for the transition, manage finding an adult primary care provider and other specialists, outline what to expect during the transition and teach medical self-advocacy skills.
